Insurance companies often require pre-authorization for medical procedures and exams. Although we are diligent in assisting patients and referring physician offices as much as possible in obtaining these authorizations, we are not ultimately responsible for ensuring pre-authorization. It is advisable to confirm that your medical procedure has been pre-authorized by calling your insurance company or checking with us ahead of your visit to confirm this.
All applicable deductibles and co-pays for services performed are collected at the time of treatments. Patients should be aware of their medical insurance benefits prior to their arrival. In order to obtain this information, call the customer service number on the back of your insurance card.
For patients with unusually high deductibles or co-pays, we may require that a deposit or payment be made to your account up-front.
